What You'll Do

Lead Patient-Centered Hospice Care

Conduct comprehensive patient and family assessments

Deliver skilled nursing care with a focus on pain and symptom management

Serve as the Care Coordinator

Act as case manager, coordinating care across the interdisciplinary team

Collaborate with physicians, social workers, chaplains, and aides

Support Patients & Families

What You Bring

Active RN license (or compact eligibility)

Graduate of an accredited nursing program
