we are seeking a highly skilled **Population Health Clinical Pharmacy Specialist** to support value-based care initiatives and improve pharmacy-related outcomes for Medicare and Commercial health plan members. This is an excellent opportunity for a board-certified clinical pharmacist passionate about population health, quality improvement, and collaborative patient care.
### Position Highlights
✅ Full-Time Permanent Position
✅ Monday–Friday Schedule (8:00 AM–4:30 PM or 8:30 AM–5:00 PM)

### Key Responsibilities
🔹 Analyze EMR and pharmacy data to improve quality measures, STAR ratings, and HEDIS outcomes
🔹 Conduct Medication Therapy Management (MTM) and patient education initiatives
🔹 Collaborate with providers to optimize medication utilization and reduce healthcare costs
🔹 Support value-based care agreements and pharmacy-related quality initiatives
🔹 Assist in preventing avoidable hospital admissions and readmissions
🔹 Develop policies, procedures, and educational programs related to pharmacy services
🔹 Serve as a pharmacy subject matter expert for population health programs
### Required Qualifications
✔ Active Pharmacist License
✔ Bachelor's Degree in Pharmacy (PharmD preferred)
✔ Board of Pharmacy Specialties (BPS) Certification Required in one of the following:
* Pharmacotherapy (BCPS)
* Ambulatory Care (BCACP)
* Cardiology (BCCP)
* Geriatrics (BCGP)
* Related Specialty Certification
✔ Minimum:
* 3 year Clinical Pharmacist experience OR
* Completion of ASHP Accredited PGY-1 Residency
