Summary

This scope of practice statement defines what physician moonlighters can do when caring for BMTCI patients at Moffitt Cancer Center. The role of physician moonlighters in BMT is to provide phone advice, medical evaluation and treatment for any BMTCI patient in the inpatient setting as well as for ambulatory patients who phone in with medical problems. The scope of these services shall be sufficient to arrive at a reasonable clinical impression and treatment plan to ensure patient safety and stability. For example: physician moonlighters can order medications to control pain, nausea, hyperglycemia, hypertension, arrhythmias, heart failure and correct electrolyte disturbances. These are all competencies within the scope of practice of a board certified internal medicine physician.Physician Moonlighter-initiated diagnosis and treatment are intended to stabilize the patient's condition and optimize safety.Physician Moonlighters are NOT expected to implement definitive or elective diagnostic or treatment measures unless these are necessary for patient safety prior to the availability of the attending physician on the premises. Non-urgent interventions should be deferred to a BMT Attending physician; patient condition permitting. In the inpatient setting, a Physician Moonlighter will call a BMTCI Attending physician to discuss any and all activities that relate to BMTCI decisions and management. For example: Physician Moonlighters cannot prescribe chemotherapy, manage immunosuppressive drugs, or treat opportunistic infections without documented discussion with an attending.The Physician Moonlighter shall contact the patient's transplant/cell therapy physician or the BMTCI Inpatient MAC, RIC or ICET Attending physician when an assessment reveals that a patient's condition has undergone deterioration or a new medical condition is suspected. This would also include a change in prognosis from baseline to a degree that indicates a medical intervention is appropriate.In the case of new inpatient admissions seen by a Physician Moonlighter, a standard history and physical shall be completed, and in all cases the patient's management shall be discussed with the patient's transplant/cellular therapy Attending, or the BMTCI MAC, RIC or ICET Service Attending MD prior to initiating any non-urgent measures.The Physician Moonlighter will communicate with the patient's Transplant/cell therapy Attending, or BMTCI MAC, RIC or ICET Attending MD as to what services/treatments were rendered so the appropriate follow up can be made.The Physician Moonlighter should promptly answer outpatient calls and provide medical advice. If an outpatient needs to be evaluated urgently, the moonlighter may elect to send the patient to a local hospital or to evaluate the patient at Moffitt. The Physician Moonlighter will need to admit certain outpatients or place them on Observation Status at Moffitt. All admissions and evaluations of outpatients need to be discussed with the patient's transplant/cell therapy physician or BMTCI MAC, RIC or ICET Attending on call.All Phone Advice must be documented in the medical record and the primary transplant/ cell therapy physician should be notified. If a Physician Moonlighter has questions about the appropriate phone advice, they shouldcontact the primary transplant/cell therapy physician or the MAC, RIC or ICET Attending on call. Finally, any outpatient who is exhibiting signs or symptoms of an acute coronary event or stroke must be referred to the closest emergency room as soon as possible. It is recommended that the patient caregiver be given instructions to call 911.Patients exhibiting signs or symptoms of an acute coronary event or stroke must not come to Moffitt.This Scope of Practice will be posted on the BMTCI Inpatient Units on 3W, 4W, 3C, and the BMT Treatment Center, and postings will be updated upon any revision(s). This document will also be made available to all Physician Moonlighters, APPs, nurses, pharmacists and any member of the team responsible for the care of BMT patients. Other applicable duties as assigned by appropriate Leadership." MOONLIGHTER-BMT JOB SUMMARY The BMTCI Physician Moonlighter practices medicine within the scope of authority and privileges granted by Moffitt Cancer Center by providing patient care under the auspices of Moffitt Medical Group; serving as member of the academic medical enterprise; and advancing the institutional mission of contributing to the prevention and cure of cancer.The BMTCI Physician Moonlighter remains in house for the duration of each shift.He/she is responsible for night and/or weekend coverage of inpatient BMTCI patients as well as assisting in fielding Triage calls from BMTCI patients from outside of hospital. The position supports patient care and patient safety.The BMTCI Physician Moonlighter will evaluate and manage patient and staff patients with the BMTCI Attending.He/she is responsible for discussing new admissions and clinical care issues that arise on inpatients during the shift with the treating inpatient team and attending physician. QUALIFICATIONS Education: MD/DO or equivalent, required Licensure/Certification: MD/DO DEA BLS/ACLS Minimum Experience Required: Satisfactory completion of an accredited internal medicine residency program. Preferred experience in the management of oncology patients and prior experience in a BMTCI in-patient clinical practice setting is preferred. JOB COMPONENTS Job Specific Duties: Provide care to BMT and ICE-T patients under the supervision of the BMT and ICE-T attendings Perform admissions, appropriate orders, progress notes, EHR appropriate orders, handoff and patient assessment and reassessment, as well as discharges as appropriate under the supervision of the BMT and ICE-T attendings Conform and adhere to all applicable policies and procedures covering physician practices and practice within BMTCI."

Salary Range

$125.00 - $125.00

Salary ranges posted for this position represent the expected base pay range for the role. Actual compensation may vary based on location and a variety of job-related factors, including experience, skills, education, and internal equity among Team Members in similar positions.
