You'll be responsible for:

Performing routine and follow-up medical evaluations

Managing chronic and acute medical conditions

Collaborating with physicians and interdisciplinary care teams

Developing individualized treatment plans

Reviewing labs, diagnostics, and medications

Communicating with patients, families, and nursing staff

Helping to prevent unnecessary hospitalizations

Active New York State Nurse Practitioner License.

DEA Registration (or ability to obtain).

Strong clinical assessment and communication skills.

Experience in: Family Medicine, Internal Medicine, Geriatrics, Skilled Nursing, Rehabilitation, Long-Term Care, Sub-Acute Care.
