Benchmark Therapy and Rehab is a large therapy practice providing services to a multitude of providers in Home Health, and we are seeking compassionate and driven Physical Therapists to join our therapy team.

Position Summary:
Physical Therapist- Home Care
Requirements and Responsibilities:

  • Evaluates and provides initial treatment with patients recovering from injury, disease, or surgery in a home care environment.
  • Establishes treatment plans using a variety of methods, including exercises, stretching maneuvers, hands-on therapy, and equipment to ease patients' pain and help them increase their mobility.
  • Works cooperatively with physicians, case managers and treating therapists.
  • Monitors, documents, and reports patients' progress and communicates needed health information regarding the patient to all appropriate health care team members.
  • Requires state license to practice physical therapy.
  • Reports to manager of clinical services.
