Your role will be primarily onsite in patients' homes within the Kissimmee area. This home care setting provides a unique opportunity to build strong relationships with patients and observe firsthand the environment in which they navigate their daily activities. Working as part of a multidisciplinary team, you will collaborate closely with other healthcare professionals to ensure that patients receive holistic and effective care tailored to their circumstances.

• Develop comprehensive and individualized treatment and care plans based on thorough assessments of patients' physical conditions and specific needs.
• Utilize various therapeutic modalities, including hands-on manual therapy techniques and therapeutic exercises, to facilitate recovery and improve mobility.
• Monitor patient progress consistently, adjusting treatment strategies as necessary, and maintain accurate and up-to-date documentation of therapy sessions and outcomes.
• Communicate openly and regularly with patients and their families regarding treatment progress, guiding them in understanding home programs and addressing any concerns that may arise.
• Exercise independent judgment as needed, leveraging your experience to implement effective treatment options in real-time situations.

As a Physical Therapist at Revive Bone Physical Therapy, you will work with a diverse range of patients, including those recovering from surgery, managing chronic pain conditions, or facing mobility challenges due to age or injury. Your ability to conduct thorough evaluations and adapt treatment plans will be crucial to meet the varying physical, emotional, and psychological needs of each individual.

• A Bachelor's degree in Physical Therapy from an accredited program.
• Active Physical Therapist license in the state of Florida.
• Minimum of three years of relevant experience in physical therapy, with a comprehensive understanding of treatment modalities and rehabilitation best practices.
• Demonstrated ability to work independently as well as collaboratively within a multidisciplinary team environment.
• Exceptional communication skills, demonstrating empathy and support towards patients and their families.

• Experience in home health physical therapy settings is highly desirable.
• Familiarity with electronic health records (EHR) and documentation standards specific to physical therapy is preferred.
• Advanced training or certifications in rehabilitation techniques or specialties are a plus.

The annual salary for this position ranges from USD 90,000 to 120,000, reflective of experience and qualifications.
