We are actively seeking a committed and empathetic Full-Time Mobile Licensed Practical Nurse (LPN) specializing in wound care to become an integral part of our healthcare team. In this role, you will dedicate your efforts to providing comprehensive wound care services across various healthcare settings, including homes, long-term care facilities, and assisted living communities. Your primary responsibility will be to assess, treat, and monitor wounds, ensuring the highest standard of care and promoting optimal healing.
Responsibilities:
Wound Assessment and Documentation: Conduct thorough assessments of patients' wounds, meticulously documenting relevant information such as size, depth, and overall condition. Maintain accurate and detailed records of wound status, treatment plans, and progress notes.
Wound Treatment and Dressing Changes: Administer prescribed wound care treatments following established protocols and physician orders. Perform dressing changes with precision, upholding aseptic technique and strict adherence to infection control standards.
Patient Education: Educate patients and their families on wound care protocols, prevention strategies, and the importance of consistent adherence to treatment plans.
Provide clear guidance on self-care for wounds, monitoring and ensuring patients' understanding and compliance.
Collaboration with Healthcare Team: Collaborate with physicians, nurses, and other healthcare professionals to develop and implement comprehensive wound care plans.
Communicate effectively to facilitate seamless coordination of care and prompt adjustments to treatment strategies.
Mobile Care Delivery: Travel to various locations, including patients' homes and healthcare facilities, to provide essential wound care services. Adapt to diverse environments and efficiently manage resources to deliver high-quality care in a mobile setting.
  1. Qualifications: Certification: Valid certification as a Licensed Practical Nurse (LPN).
  2. Specialization: Additional certification or training in wound care management.
  3. Experience: Previous experience in wound care highly preferred.
  4. Communication Skills: Excellent verbal and written communication skills.
