Position Summary

We are seeking a detail-oriented and experienced MDS Coordinator to oversee the completion of Minimum Data Set (MDS) assessments and ensure accurate clinical reimbursement and regulatory compliance. The MDS Coordinator plays a critical role in care planning, interdisciplinary coordination, and maintaining survey readiness.

Key Responsibilities
  • Complete and submit accurate and timely MDS assessments in accordance with CMS regulations

  • Coordinate interdisciplinary care plan meetings

  • Ensure compliance with federal and state regulations

  • Monitor case mix index (CMI) and reimbursement processes

  • Review documentation to ensure clinical accuracy and support for coding

  • Educate nursing staff on documentation requirements

  • Participate in Quality Assurance and Performance Improvement (QAPI) initiatives

  • Assist with survey preparation and response

Qualifications
  • Active RN license

  • MDS experience in long-term care required
