Position SummaryWe are seeking a detail-oriented and experienced MDS Coordinator to oversee the completion of Minimum Data Set (MDS) assessments and ensure accurate clinical reimbursement and regulatory compliance. The MDS Coordinator plays a critical role in care planning, interdisciplinary coordination, and maintaining survey readiness.
Key ResponsibilitiesComplete and submit accurate and timely MDS assessments in accordance with CMS regulations
Coordinate interdisciplinary care plan meetings
Ensure compliance with federal and state regulations
Monitor case mix index (CMI) and reimbursement processes
Review documentation to ensure clinical accuracy and support for coding
Educate nursing staff on documentation requirements
Participate in Quality Assurance and Performance Improvement (QAPI) initiatives
Assist with survey preparation and response
Active RN license
MDS experience in long-term care required
