Key Responsibilities

  • Collaborate closely with interdisciplinary teams, including medical staff and other health professionals, to coordinate comprehensive care plans that prioritize the clients' well-being.
  • Advocate for clients by empowering them to access community resources and support services, enhancing their ability to cope with mental health challenges.

Patient Population / Client Focus

The primary focus of this role will be on clients receiving hospice care, which may include individuals facing terminal illnesses or significant life transitions. You will provide emotional and psychological support to help clients and their families navigate these challenging experiences, emphasizing the importance of mental well-being during end-of-life care.

Qualifications
  • A Master's degree in Social Work from an accredited program is required.
  • Must hold current licensure as a Licensed Clinical Social Worker (LCSW) in California.
  • A minimum of two (2) years of post-licensure experience delivering clinical social work services.
  • Proficient in conducting diagnostic assessments and developing tailored treatment plans.
  • Demonstrate strong interpersonal and communication skills, with proven ability to connect with clients from diverse backgrounds.

Compensation and Benefits
