Key Responsibilities

  • Engage in thorough follow-up assessments and create prevention plans following crises, ensuring continuity of care through collaboration with outside agencies and providers.
  • Maintain accurate clinical documentation and administrative records that comply with contractual and billing requirements, submitting encounter forms on a weekly basis to facilitate effective service delivery.
  • Participate regularly in weekly clinical meetings to promote communication and integration between primary care and mental health services, sharing insights and strategies to improve patient outcomes.
Patient Population / Client Focus
