Key Responsibilities
  • Conduct crisis intervention and triage for patients in need, performing assessments and de-escalation techniques to stabilize situations effectively. If a crisis escalates, you will coordinate with the public mental health crisis response team as required.
  • Engage in thorough follow-up assessments and create prevention plans following crises, ensuring continuity of care through collaboration with outside agencies and providers.
  • Maintain accurate clinical documentation and administrative records that comply with contractual and billing requirements, submitting encounter forms on a weekly basis to facilitate effective service delivery.
  • Participate regularly in weekly clinical meetings to promote communication and integration between primary care and mental health services, sharing insights and strategies to improve patient outcomes.
Patient Population / Client Focus

Your primary focus will be on providing mental health services to disadvantaged populations, highlighting the importance of culturally competent care. You will address a variety of mental health issues, including depression and anxiety, especially in communities struggling with access to quality care.
