Tranquil Haven Counseling is seeking a dedicated Licensed Marriage and Family Therapist (LMFT) to join our compassionate team, providing essential mental health services to a unique patient population in a hospice setting. Work Environment

This role is entirely remote, providing you with the flexibility to balance work responsibilities while maintaining a healthy work-life dynamic. As part of our dedicated team, you will collaborate with healthcare professionals, including physicians, nurses, and social workers at the hospice center, to create individualized treatment plans that support each patient’s journey. Although most interactions occur virtually, you will have the opportunity to participate physically in select meetings and community outreach events.

Key Responsibilities
  • Conduct in-depth assessments and mental health screenings to identify the emotional and psychological needs of patients and develop tailored treatment plans.
  • Engage in short-term individual counseling and crisis intervention for patients facing terminal illnesses, grief, or existential concerns, using evidence-based practices.
  • Facilitate group therapy sessions and workshops addressing relevant topics such as coping strategies, mindfulness, and family dynamics during end-of-life care.
  • Collaborate with interdisciplinary teams, including medical professionals and support staff, to strategize holistic care approaches that enhance patient outcomes.
  • Implement outreach initiatives aimed at raising awareness about mental health care in hospice settings and provide resources for patients and families.
  • Maintain meticulous, confidential patient records in accordance with ethical and legal standards, utilizing electronic health records (EHR) technology.
Patient Population / Client Focus

As a Licensed Marriage and Family Therapist at Tranquil Haven Counseling, you will serve a diverse patient population primarily within the hospice sector. Qualifications
