These services can include nursing care, physical therapy, occupational therapy, and assistance with daily activities. The goal is to help patients recover, maintain their independence, and avoid unnecessary hospitalizations. Mercy Home Health Care is delivered by licensed professionals and tailored to each patient's specific needs, often coordinated with the patient's physician.

Collaboration is key—professionals regularly communicate with patients’ families to ensure care plans are followed and to address any concerns. They also coordinate with physicians, therapists, and other healthcare team members to provide holistic and consistent care, making adaptability and strong communication skills essential.
