Are you passionate about supporting seniors with memory care needs? Legacy House of Southern Hills is on the lookout for a dedicated and experienced full-time Memory Care Program Coordinator to take the lead in our Memory Care unit.

WHAT WILL YOU DO AS OUR MEMORY CARE PROGRAM COORDINATOR?
  • Care Assessment & Planning: Conduct initial and ongoing evaluations of residents' cognitive and physical abilities and develop individualized care plans in collaboration with the care team.
  • Team Leadership: Supervise care staff to ensure high-quality support is provided and train team members on effective memory care techniques and communication methods.
  • Resident Engagement: Lead daily activities that promote cognitive health and social connection, including individual and group programs tailored to memory care needs.
  • Family Support: Act as the primary contact for families, offering regular updates and providing resources to help them navigate memory-related challenges.
  • Documentation & Compliance: Maintain detailed and accurate records of resident progress and care and participate in meetings to review and adjust care strategies.
  • Healthcare Collaboration: Partner with medical professionals to coordinate integrated care and ensure services meet regulatory standards and memory care best practices.
  • Environmental Oversight: Monitor and adjust the memory care environment to ensure it is safe, secure, and supportive of residents' well-being and independence.

WHEN WILL YOU WORK?
This full-time position offers a consistent Tuesday through Saturday schedule.
WHAT DO YOU NEED TO BE OUR MEMORY CARE PROGRAM COORDINATOR?
  • Medication Technician certification
  • Experience in healthcare or social services, particularly in gerontology or memory care
  • Knowledge of memory disorders and understanding of therapeutic interventions
  • Ability to manage a team and promote a positive, supportive environment
  • Strong communication and interpersonal skills
