Emplify Health by Gundersen is looking for a full-time Medication Reconciliation Tech. The Medication Reconciliation Tech supports the pharmacy department by collecting accurate medication histories, identifying barriers to medication adherence, and assisting with medication reconciliation and safe transitions of care. This role requires strong communication, problem-solving, and motivational interviewing skills, along with the ability to work collaboratively and support high standards of patient care.
What you will do/job specifics
  • 1.0 FTE (40 hours/week)
  • 8-hour shifts, AM and PM shifts
  • You will start by working the AM's transitioning to include PM's and weekends
  • La Crosse, WI

What's Required/What you need
  • High school diploma or equivalency
  • 2-years of related healthcare experience requiring basic pharmacy knowledge and skills
  • Registration as a Pharmacy Technician in WI
  • National-recognized certification for Pharmacy Technician credential (CPhT)
  • Achievement of Medication History Certificate through Pharmacy Technician Certification Board (PTCB) or Board of Pharmacy Technician Specialties (BPTS) within 12 months of hire
