Registered Nurse
Location: Lakewood, CO
Job Type: Full-time
Responsibilities
  • Complete SOC, ROC, Recertifications, supervisory visits, and any necessary revisits based on patient needs and clinician coverage.
  • Manage an anticipated weekly workload of approximately 10-12 SOC/ROC/Recert visits or the equivalent (2 revisits = 1 SOC/ROC/Recert).
  • Complete all required documentation in a timely manner according to agency and Medicare guidelines.
  • Manage and provide patient care while performing RN responsibilities within scope of practice.
  • Supervise LPNs in assigned facilities and manage nursing visit utilization.
  • Coordinate and oversee care for assigned accounts/territories by:
    • Attending resident reviews.
    • Maintaining consistent communication with facility staff.
    • Collaborating with marketers and account managers to support census growth.
    • Reviewing fiscal outcomes of cases/accounts.
    • Working with hospice clinical staff and facility staff to coordinate additional patient resources, including hospice transitions, private caregivers, and care recommendations.
  • Manage patient cases by ensuring appropriate utilization of clinical disciplines.
  • Prepare fiscal reports on assigned cases as requested by leadership.
  • Attend relevant care coalition meetings (e.g., SDCC) as assigned.
  • Work a minimum of 40 hours per week, typically Monday through Friday, with hours varying based on patient and account needs.
Qualifications
  • Graduate of an accredited school of professional nursing.
  • Active Registered Nurse license in the State of Colorado in good standing.
  • Ability to communicate effectively with patients and document patient-related activities accurately.
  • Current driver's license.
  • Minimum of one (1) year of experience as a Registered Nurse.
  • Home Health experience preferred.
  • Familiarity with OASIS documentation preferred.
