RN Case Manager - Care Management
Location: Colorado
Contract Length: 8 Weeks
Position: Full-Time
Guaranteed Hours: 36 hours/week
Start: Quick Start

  • Weekend coverage: 5 hours one Saturday per month
  • The 5 weekend hours will be offset by 5 hours off during the week
Job Summary
The RN Case Manager is responsible for coordinating, monitoring, and evaluating patient care to ensure appropriate, effective, and cost-efficient services. This role works closely with patients, physicians, healthcare providers, payers, and interdisciplinary teams.
Responsibilities
  • Assess and coordinate patient care needs throughout the healthcare continuum.
  • Develop, implement, and evaluate case management plans.
  • Coordinate discharge planning and appropriate levels of care.
  • Collaborate with physicians, patients, families, staff, and insurance payers.
  • Monitor utilization of healthcare resources and ensure appropriate patient care.
  • Document case management activities clearly and accurately.
  • Educate physicians and staff regarding utilization and level-of-care issues.
  • Participate in departmental and interdisciplinary meetings.
  • Maintain knowledge of case management, discharge planning, utilization review, and nursing practices.
  • Support quality, operational, and financial outcomes.
Requirements
  • Current unrestricted Colorado RN License required
  • Bachelor's Degree in Nursing required; Master's preferred
  • Minimum 3 years of clinical hospital experience
  • Minimum 5 years of hospital case management experience preferred
  • Current BLS certification
  • Case Management certification preferred, or willingness to obtain certification within 2 years
  • Strong knowledge of discharge planning, utilization management, and continuum of care
  • Excellent communication, documentation, and collaboration skills
