Experienced Clinician Wanted – We Will Train!
Do you possess outstanding clinical skills and a dedication to providing exceptional patient care? We are seeking talented professionals and are willing to provide training and support for the right individual.

Position Summary
For over 40 years, North Hill, a not-for-profit senior living community located in Needham, Massachusetts, has provided vibrant living opportunities to people 65+ by offering a host of hospitality, wellness and health care services. The Registered Nurse is Responsible for maintaining the delivery of quality patient care by assuring that the Patient Plan of Care is followed. The position combines the activities of direct and indirect patient care and provides direct care to adult and geriatric patients. The Registered Nurse directs Certified Home Health Aides and Certified Nursing Assistants in patient care tasks, performs patient assessments, develops patient care plans, evaluates the plan effectiveness, modifies it as appropriate and documents all patient care provided.

  • Knowledge of Medicare Conditions of Participation as well as State and federal home health care regulations
  • Responsible for patient evaluation, assessment, treatment, preventative care, patient teaching, and documentation of services provided according to a physician’s order plan
    of care
  • Evaluates the effectiveness of plan of care and collaborates with interdisciplinary team members to provide high quality clinical care
  • Participates in weekly interdisciplinary care plan meetings
  • Responsible to document visits and communication within electronic medical record within 24 hours of occurrence
  • Participates in infection control meetings and quality performance initiatives, Quality Assurance and Performance Improvement meetings
  • Communicates concerns with immediate supervisor and works with the interdisciplinary team to meet resident needs
  • Anticipates potential problems, implements interventions and communicates per documentation guidelines
  • Responsible to supervise ancillary staff according to the Medicare Conditions of Participation
  • Participates in discharge planning and referring to appropriate community resources with physician follow-up
  • Follow all Emergency Management Plan and actively participates in fire and evacuation
    drills
  • Meets department uniform, appearance and grooming requirements
  • Complies with all OSHA safety regulations, local applicable laws regarding health, safety or environment

Required Skills and Abilities
  • Ability to maintain confidentiality and strong customer support skills
  • Complies with protection of health information (HIPAA) safeguards
  • Ability to work independently with limited supervision
  • Work professionally with people from a wide variety of cultural and socio-economic backgrounds
  • Must possess the ability to deal tactfully with Team Members, Residents, family members, visitors, and the general public
  • When working in Resident areas, must be able to communicate effectively in English
  • Attends and participates in in-service educational classes, Healthcare Academy and staff meetings as directed

Education and Experience
  • Bachelors of Science in Nursing, Registered Nurse preferred
  • Professional license in good standing in the Commonwealth of Massachusetts
  • Ability to read, write and understand English
  • 2 years’ experience as a Registered Nurse, preferably in a home health care setting
  • Proficient with computer software programs
Satisfactory completion of a background investigation is required for this position.
