Job description

At EPIC Health, we're committed to providing exceptional care to our patients, and we're currently seeking an outstanding Health Navigator to join our team. In this role, you'll be responsible for providing patient-centered care coordination and assistance to patients in navigating the complex healthcare system.

As a Health Navigator, you'll work closely with patients to understand their individual needs, identify potential barriers to care, and develop personalized care plans. You'll also help patients navigate the healthcare system by scheduling appointments, coordinating referrals to specialists, and assisting with insurance and billing issues.

The ideal candidate for this position will have a deep commitment to patient care, exceptional communication skills, and a passion for making a difference in the lives of others. You'll need to be a detail-oriented, proactive problem solver who can manage multiple tasks and priorities in a fast-paced environment.

Key responsibilities of the Health Navigator role include:

  • Building relationships with patients and their families to better understand their healthcare needs and provide compassionate support
  • Assisting patients in navigating the healthcare system by scheduling appointments, coordinating referrals to specialists, and managing insurance and billing issues
  • Coordinating care with other healthcare providers and community resources to ensure patients receive comprehensive, integrated care
  • Providing patient education and support to help patients better manage their conditions and improve their overall health and wellness
  • Conducting assessments to identify patients' social determinants of health and addressing these factors in the care plan
  • Documenting patient interactions, care plans, and progress in electronic medical records
  • Participating in quality improvement activities to continuously improve the delivery of care and patient outcomes

Qualifications for this role include:

  • Current active licensure as an LPN, EMT, or Health Educator in Case Management
    OR
    A Bachelors Degree in Psychology, Social Work, Exercise Science, health administration, or other related field
    OR
    2+ years Experience working as a medical assistant
    OR
    Currently pursuing a degree in pre-med, pre-nursing, or other related field
  • Experience with patient-centered care coordination, case management, or care navigation or just an overall desire to help people
  • Excellent communication and interpersonal skills, with the ability to build rapport and trust with patients and their families
  • Strong organizational and time-management skills, with the ability to manage multiple priorities and tasks effectively
  • Familiarity with electronic medical records and other healthcare technology tools
  • Knowledge of healthcare policies and regulations, including Medicare and Medicaid
