What You'll Do

  • Develop and maintain relationships with physicians, hospitals, rehabilitation facilities, skilled nursing facilities, assisted living communities, case managers, and other referral sources.
  • Increase home health admissions and achieve monthly sales goals.
  • Execute strategic territory management plans.
  • Educate referral partners on Concierge Home Care's services.
  • Coordinate smooth patient transitions from hospital or facility to home.
  • Serve as a trusted healthcare resource within the community.
  • Collaborate closely with clinical leadership to ensure outstanding patient outcomes.
  • Maintain CRM documentation and referral activity.
  • Represent Concierge Home Care at networking events and community outreach opportunities.


Qualifications

Required

  • Healthcare sales experience or strong clinical experience with existing healthcare relationships
  • Excellent communication and relationship-building skills
  • Self-motivated with the ability to work independently
  • Valid driver's license
  • Reliable transportation
