Job Summary:

As a Program Coordinator at Firefly, you will play a vital role in overseeing and coordinating operations within designated service delivery areas, ensuring that individuals receive the highest quality of care. We are looking for someone who is organized, detail-oriented, and passionate about supporting individuals with developmental disabilities. Your leadership will help foster a supportive environment that encourages personal growth and community participation.

In this role, you will be responsible for the development and implementation of Individual Support Plans (ISPs), ensuring that outcomes are measurable and clear. You will also supervise and evaluate team members, maintain accurate documentation, and coordinate medical care to promote wellness.

Key Responsibilities:

  • Coordinate and participate in the development of the Individual Support Plan (ISP) to ensure clear and measurable outcomes.

  • Supervise and evaluate team members, ensuring they are trained in the individual's written plan and their responsibilities.

  • Conduct quarterly Record and Therapy Reviews to ensure documentation is current and complete.

  • Make weekly visits to the ISL to monitor individual status and ensure a safe and clean environment.

  • Maintain client profiles in the database and prepare annual ISL budgets.

  • Track monthly ISL budget exceptions and prepare end-of-year variance reports.

  • Collaborate with Primary Support Staff and Residential Nurse to address medical needs and promote wellness.

  • Complete daily documentation audits in Therap, including health, medications, and daily notes.

  • Facilitate opportunities for natural support and community participation.

  • Other duties as assigned.

Education, Experience, and/or Credential Qualifications:

  • A bachelor's degree from an accredited college or university.

  • Relevant experience in the field of Developmental Disabilities can be substituted year for year for a degree.

Additional Qualifications:

  • Must have a reliable means of communication.

  • Must possess skills to read and understand written materials.

  • Must possess skills to write clearly and legibly.

  • May be required to assist with physical transfers or lifting, depending on the needed supports of the individual.

Physical Requirements:

  • Sedentary work: Exerting up to 10 pounds of force occasionally and/or a negligible amount of force frequently to lift, carry, push, or pull, or otherwise move objects, including the human body.

  • Repetitive movements of hands, fingers, and arms for typing and/or writing during work shift.

  • Jobs are sedentary if walking and standing are required only occasionally, and all other sedentary criteria are met.
