Job description

UW Health is seeking a Pharmacist for a job in Middleton, Wisconsin.

Job Description & Requirements

Work Schedule
100% FTE, day shift. Monday through Friday with hours varying between the hours of 6:00AM – 5:00PM. Weekend rotation every 6-7 weeks: Saturday 8:00AM – 4:30PM. You will work at the Pharmacy Services Building in Middleton, WI. Hours may vary based on the operational needs of the department.

We are seeking a Clinical Pharmacist – Specialty Pharmacy Onsite to:

  • Be responsible for ensuring the safe, accurate, and compliant dispensing of specialty medication within a warehouse-based pharmacy operation
  • Management of Risk Evaluation and Mitigation Strategy (REMS) program requirements, overseeing fulfillment of specialty medications for bleeding disorders, triaging medication orders from call center
  • Work collaboratively with other pharmacists and pharmacy technicians to ensure timely medication access while maintaining regulatory compliance and the highest standards of patient safety

Qualifications

  • Bachelor's Degree in Pharmacy or a PharmD degree Required
  • Completion of an ASHP Accredited Residency training program Preferred

Work Experience

  • 1 year of clinical patient care and orders management experience Required
  • 3 years of institutional experience (hospital or integrated health system) or completion of an ASHP accredited residency training program Preferred

Licenses & Certifications

  • Licensed to practice pharmacy in the State of Wisconsin within 90 Days Required
  • Basic Life Support (BLS) within 90 Days Required
  • Certification through the appropriate certifying program (Board of Pharmacy Specialties, National Board of Nutrition Support Certification, National Certification Board for Anticoagulation Providers, Specialty Pharmacy Certification Board, etc.) Preferred
