• Practice Setting: Free-Standing Cancer Center
• Reason for Coverage: Loss of provider
• Dates of Coverage: November 1, 2025 to perm ideally
• Call Schedule: No call during locum period; call expected if placed perm; call schedule is 1 week out of every 6-8 weeks
