Job description

JOB SUMMARY:

This position is a member of the interdisciplinary medical team of Family Health Center of Southern Oklahoma, Inc. He/She assists medical providers in the examination and treatment of patients and functions as a liaison between the medical staff, health information, laboratory and administrative departments.

MA RESPONSIBILITIES:

  1. Assists medical providers with examination and treatment of patients.
  2. Maintains first line contact with all patients who come into vital signs area.
  3. Performs routine intake procedures such as height, weight, temperature, pulse and blood pressure readings.
  4. Assists with laboratory and office procedures.
  5. Responsible for assisting nursing staff with triage of patients and transfer of charts to appropriate provider.
  6. Assists medical providers with examination and treatment of patients.
  7. Relieves other departments as indicated as time permits.
  8. Provides assistance with administrative procedures required, including patient referral, charting/documentation activities and telephone calls.
  9. Attends clinic-wide and departmental in-services and training.
  10. Demonstrates a spirit of teamwork and commitment to quality patient care.
  11. Performs other related duties incidental to the work described herein.
  12. Screens patients for visual acuity using Snellen method as needed.
  13. Obtains routine temperature, pulse, respiration, height and weight as well as blood pressure readings at first encounter.
  14. Responsible for routine laboratory procedures including but not limited to: Phlebotomy, various dipsticks, urinalysis as directed by medical staff.
  15. Performs other related duties incidental to the work described herein.
  16. Responsible for routine x-rays, EKG's and BioTE.
  17. Responsible for ordering patient medications.
  18. Supports and participates with activities related to the Health Disparity Collaborative. Duties include but are not limited to obtaining patient information for data entry, participation in team meetings, production and submission of monthly reports and maintenance of database.
  19. Participates in provider care team meetings to comply with PCMH/Performance Improvement (P.I.), and Accountable Care Organization standards.
  20. Participates in Accountable Care Organization processes/meetings to meet standards for Medicare/Medicaid shared savings reimbursement.
  21. Participates in Uniformed Data Systems processes/meetings to meet standards for proper documentation and data collection according to Uniformed Data System Criteria.
  22. Participates in designated activities and projects for PCMH as assigned by the Medical Director.
  23. Maintains up to date required certificates.
  24. Follows the mission and vision statement.
  25. All other duties as assigned.

LPN RESPONSIBILITIES:

  1. Become familiar and proficient with electronic medical records software.
  2. Interviews patient to determine medical problem/condition and documents in chart for physician.
  3. Obtains and records patient's vital signs and weight.
  4. Assists physician with patient examination as needed.
  5. Reviews physician's orders, lab requests, or follow-up needs with patient.
  6. Documents services performed for billing purposes.
  7. Dresses wounds
  8. Applies compresses
  9. Observes patients and reports adverse reactions to medication or treatment to medical personnel in charge.
  10. Administers specified medication, orally or by subcutaneous, intravenous, or inter-muscular injection, and notes time and amount on patients' charts.
  11. Assembles and uses such equipment as catheters, tracheotomy tubes, oxygen suppliers, nebulizers, and IV's.
  12. Collects samples, such as urine, blood, and sputum, from patients for testing
  13. Sterilizes equipment and supplies.
  14. Cleans rooms
  15. Maintains supplies, including stocking and ordering new supplies and disposing of expired or damaged supplies.
  16. Responds to patient telephone inquiries by evaluating and responding to the patient's problems and questions.
  17. Follows up with patients on lab results, including the scheduling of any additional tests.
  18. Coordinates referrals for patients through insurance and other physician offices.
  19. Participates in provider care team meetings to comply with PCMH/Performance Improvement (P.I.), and Accountable Care Organization standards.
  20. Participates in Accountable Care Organization processes/meetings to meet standards for Medicare/Medicaid shared savings reimbursement.
  21. Participates in Uniformed Data Systems processes/meetings to meet standards for proper documentation and data collection according to Uniformed Data System Criteria.
  22. Participates in designated activities and projects for PCMH as assigned by the Medical Director.
  23. Becomes familiar with the process of documentation as well as screening for medical encounters.
