A premier orthopedic group is seeking a motivated and skilled Outpatient Physical Therapist to deliver outpatient-level care directly in patients’ homes. This unique position offers the opportunity to collaborate closely with orthopedic surgeons and the hospital’s management company, ensuring a streamlined, high-quality care experience from surgery through recovery.
Duties
- Provide evidence-based physical therapy services to post-surgical and orthopedic patients in their homes.
- Collaborate with surgeons, physician assistants, and care managers to ensure continuity of care.
- Monitor patient progress and adjust treatment plans as necessary to ensure optimal outcomes.
- Educate patients and their families about treatment options, exercises, and preventive measures.
- Conduct comprehensive evaluations and implement tailored treatment plans
- Document all care in alignment with outpatient standards and regulatory requirements
- Participate in ongoing case reviews and care team meetings
- Work with and supervise PTAs
Experience & Qualifications:
- Current and valid Physical Therapist license in TX
- Minimum 1 year of outpatient orthopedic or home health experience preferred but not required
- If new grad, will provide training and mentorship
- Strong interpersonal and communication skills
- Self-directed, with the ability to work independently in the field
- Reliable transportation and willingness to travel within service area
