Job description

Job Title: Certified Home Health Aide - Caregiver
Department: ElderONE- North Park
Location: ElderONE - North Park Pace Center, 355 N Park Dr, Rochester, NY 14609
Hours Per Week: 24
Schedule:Rotating, 7:00am, 3:00pm, 11:00pm, every other weekend every other holiday

SUMMARY:

As a Certified Home Health Aide (HHA), you will provide care to clients in their home, following their plan of care. You make a positive difference each and every day by demonstrating care and compassion to our patients.

This position has a designated home base facility and may require occasional travel to additional locations. Typically, travel is limited to one to two locations per day, with up to four sites in Rochester, NY and one in Newark, NY. Mileage reimbursement is provided.

RESPONSIBILITIES:

  • Patient Care & Service - Promote and restore patients' health by completing all activities listed in the care plan; collaborate with Community Health Nurse/Therapist; perform various activities such as personal care, light housekeeping, meal preparation and errands.
  • Communication - Maintain effective communication to convey patient health status, treatment plans and progress; communicate with office and supervisory staff regularly.
  • Documentation - Accurately and completely document client care on activity sheet.
  • Compliance - Adhere to required department and system protocols, HIPPA regulations, local, state, federal requirements.

Minimum Qualifications:

Candidates must meet one of the following:

  • Active Home Health Aide (HHA) certification

  • HHA with a lapse of more than two years of employment (must complete written exam and 1-day HHA Skills Competency Class within 30 days of hire)

  • Certified Nursing Assistant (CNA) willing to complete a written exam and 1-day HHA Skills Competency Class within 30 days of hire

  • Out-of-state HHA or Nurse Aide training with documented competency evaluation

  • Nursing student (Fundamentals of Nursing completed within the past 24 months)

  • U.S. Military-trained medic or medical technician

Preferred Qualifications:

  • High school diploma or equivalent

  • 1 year experience working with the frail elderly population

  • Compassionate, patient focused and dependable professional

  • Critical thinking skills, decisive judgment and the ability to work with minimal supervision

  • Exceptional documentation and communication skills

  • Ability to follow verbal and written instructions

EDUCATION:

LICENSES / CERTIFICATIONS:

NYS DL - valid New York State Driver's License - New York State Department of Motor Vehicles (NYSDMV)

PHYSICAL REQUIREMENTS:

M - Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects; Requires frequent walking, standing or squatting.
