Join our team at Potomac Valley Rehabilitation and Healthcare Center, a 175-bed facility renowned for its exceptional rehabilitation, skilled nursing, palliative care, and behavioral health programs. We are seeking a dedicated and detail-oriented MDS Coordinator to work with our interdisciplinary team to deliver high-quality care to our patients.
About the Role:
As an MDS Coordinator, you will play a crucial role in ensuring accurate and timely submission of Minimum Data Set (MDS) assessments and care plans for our patients. You will collaborate with our healthcare team, including physicians, nurses, and therapists, to gather and document patient information, and ensure compliance with federal and state regulations.
Responsibilities:
  • Coordinate and conduct MDS assessments, care plans, and care area assessments
  • Gather and document patient information, including medical, social, and functional data
  • Ensure accurate and timely submission of MDS assessments and care plans
  • Collaborate with healthcare team members to gather and document patient information
