Position Summary

We are seeking an experienced and detail-oriented MDS Coordinator (Registered Nurse) to join our team in Maryland. The MDS Coordinator is responsible for overseeing the Resident Assessment Instrument (RAI) process, ensuring accurate and timely completion of MDS assessments, care planning, and compliance with state and federal regulations.


Key Responsibilities
  • Coordinate and complete Minimum Data Set (MDS) assessments in accordance with CMS guidelines
  • Ensure timely and accurate submission of MDS assessments
  • Develop, implement, and update individualized care plans in collaboration with the interdisciplinary team
  • Monitor documentation to ensure compliance with regulatory standards
  • Participate in Medicare and Medicaid reimbursement processes
  • Conduct care plan meetings with residents and families
  • Educate and support nursing staff on MDS processes and documentation requirements
  • Perform audits and quality assurance reviews related to MDS and care planning
  • Stay current with changes in regulations and best practices

Qualifications
