We are seeking an experienced Director of MDS to lead the MDS program at a skilled nursing facility in Rochester, NY.

This is an important leadership role for a strong MDS professional who understands reimbursement, compliance, documentation, and the impact the MDS department has on the overall performance of a facility.

Responsibilities
  • Oversee the full MDS process and ensure all assessments are completed accurately and on time

  • Ensure compliance with all state and federal MDS requirements

  • Monitor CMI, reimbursement, and opportunities to improve documentation and capture

  • Lead interdisciplinary care plan meetings

  • Work closely with nursing, therapy, administration, and other department heads

  • Identify and address documentation gaps that may impact reimbursement or compliance

  • Track deadlines and maintain an organized, accurate MDS schedule
