Job description

Batavia Rehabilitation and Nursing is looking for a strong MDS Coordinator. This position is part time with flexibility on days and hours. This position requires a valid and current LPN or RN NYS license.

The primary responsibility of an MDS coordinator is to manage and assess patient care, to ensure the facility provides consistent quality care.

Job Responsibilities

  • Coordinate the RAI process, schedule all OBRA required and PPS assessments for all assigned residents according to state and federal regulations

  • Review resident for 30 days following Medicare non-coverage

  • Coordinate input of MDS and care plan data into computer for electronic submission according to regulations

  • Schedule Medicare MDS to maximize reimbursement data analysis

  • Review resident record for skilled nursing needs in accordance with Medicare A guidelines

  • Schedule and participate in care plans

Qualifications:

  • Minimum of 2 years of experience in long-term care

  • Valid LPN or RN License

  • Exceptional communication and interpersonal skills to foster positive relationships with residents, families, and staff.
