Job description

Position Summary
The MDS Nurse (LPN), under the direction of the Director of Nursing (DON) is responsible for coordinating the Resident Assessment Instrument (RAI) process including completion of an accurate Minimum Data Set (MDS) and collection of data to develop the Interdisciplinary Plan of Care.
Posted Salary Range
USD $36.00 - USD $42.00 /Hr.
Wage Commensuration
The wage range for this role takes into account the wide range of factors that are considered in making compensation decisions including, but not limited to, skill sets, experience, education and training, licensure and certifications, and other business and organizational needs.
Duties & Responsibilities
  • Coordinating the Resident Assessment Instrument (RAI) process
  • Completing an accurate Minimum Data Set (MDS) and development of the interdisciplinary Plan of Care.
  • Participating in the daily Medicare Prospective Pay System (PPS) Interdisciplinary Team Meeting (ITM).
  • Establishing and maintaining positive relationships with all members of the healthcare team and will function as part of the nursing management team.

Qualifications & Requirements
  • Must have a current, unencumbered LPN license (as required by state)
  • 1+ years of direct hands-on nursing and MDS/RAP experience preferred.
  • Must have the ability to utilize both local and consultant resources in the execution of job responsibilities.
  • Must have the ability to develop an adequate knowledge of state, federal, and regulatory requirements related to the job.
  • Reliable transportation required
