Location: Southview Acres Healthcare Center - St. Paul, MN
Employment Type: Full-Time
Position Summary:
Southview Acres Healthcare Center is seeking a detail-oriented and experienced MDS Coordinator to join our clinical leadership team. The MDS Coordinator is responsible for coordinating and completing the Resident Assessment Instrument (RAI) process in accordance with federal and state regulations, ensuring accurate reimbursement, and supporting high-quality resident care.
Key Responsibilities:
  • Coordinate and complete MDS assessments (including OBRA, PPS, and any applicable state-specific assessments)
  • Ensure accuracy and timeliness of all MDS documentation and submissions
  • Collaborate with interdisciplinary team members to develop and update comprehensive care plans
  • Monitor case mix index (CMI) and identify opportunities for optimization
  • Ensure compliance with CMS guidelines and Minnesota state regulations
  • Participate in care plan meetings and communicate effectively with residents and families
  • Assist with audits, surveys, and quality improvement initiatives
  • Stay up to date on regulatory changes and reimbursement updates

Qualifications:
  • Current RN license in the state of Minnesota
  • Minimum 1-2 years of MDS experience preferred
  • Strong knowledge of RAI process, ICD-10 coding, and PDPM
  • Experience in a skilled nursing facility (SNF) or long-term care setting
