Key Responsibilities
  • Conduct ongoing assessments for clients to determine mental health needs and appropriate treatment plans, tailoring interventions to individual circumstances.
  • Effectively communicate with supervisors, caregivers, and family members to provide updates on client progress, ensuring a cohesive support network.
  • Maintain an organized schedule of appointments, home visits, and clinical observations to monitor clients' therapeutic advancements and adherence to program plans.
  • Coordinate home visits to encompass in-home observations, thorough chart reviews, and insightful staff interviews, ensuring a comprehensive understanding of each clientโ€™s environment.
  • Implement and monitor the effectiveness of program plans, ensuring that interventions are not only delivered but are also producing meaningful outcomes for clients.
  • Participate in interdisciplinary team meetings to provide insights and collaborate on treatment strategies that enhance client care.
  • Prepare and maintain accurate monthly and quarterly progress notes, documenting client developments and areas for continued intervention.
  • Lead training sessions to ensure that all team members are well-versed in program methodologies and documentation procedures.
