We are seeking a dedicated Licensed Clinical Social Worker (LCSW) to join our remote team at Great Lakes Psychology Clinic, specializing in hospice care. This position is entirely remote, granting LCSWs the flexibility to work from home while maintaining essential face-to-face interactions via telehealth platforms. You will collaborate with a multidisciplinary team of professionals, including physicians, nurses, and chaplains at our hospice center, to create a unified support system for our patients and their families.

• Conduct thorough assessments of clients’ mental health and social needs, gathering relevant history and current challenges to inform diagnosis and treatment plans.
• Utilize evidence-based therapeutic modalities such as CBT, DBT, and mindfulness therapy to provide effective psychotherapy tailored to individual client needs.
• Facilitate the development of personalized treatment plans in collaboration with clients, ensuring that measurable goals and interventions are outlined and regularly reviewed for progress.
• Act as an advocate for clients, assisting them in navigating healthcare systems, securing social services, and accessing essential resources to improve their quality of life.
• Document all patient interactions, treatment plans, and progress notes meticulously to comply with legal regulations and facilitate continuity of care.
• Participate in crisis intervention efforts, utilizing de-escalation techniques and supportive counseling in high-stress situations.
• Conduct support groups and educational sessions for families and caregivers, fostering a supportive community environment.
• Engage in regular supervision and professional development activities to stay current with advancements in mental health practices and methodologies.

The ideal candidate will work primarily with hospice patients and their families, who face emotional and psychological challenges associated with terminal illness. You will play a crucial role in providing emotional support, grief counseling, and resources to ensure that patients and their loved ones cope effectively during this critical stage of life.

• A Master’s degree in Social Work from an accredited institution is required.
• Current state licensure as a Licensed Clinical Social Worker (LCSW), demonstrating your fulfillment of professional standards.
• A minimum of three years of post-master’s experience in the mental health field, specifically in providing psychotherapy services.
• Proven expertise in solution-focused techniques to help individuals achieve therapeutic milestones effectively.
