Job description

Position Summary:

This professional management role is responsible for implementing the agency’s mission while embodying its Core Values in collaboration with supported participants, their families, friends, and their evolving social networks.

Duties & Responsibilities:

Essential Job Duties:

Ensure that participants who receive services are supported in realizing their aspirations and goals.  

  1. Respect the choices of participants and provide information, encouragement, and guidance in the exercise of those choices.
  2. Ensure that participants who receive services have opportunities to develop meaningful relationships and that they receive assistance, modeling, coaching, and support in doing so. 
  3. Ensure that participants who receive services have opportunities to develop membership in social groups and communities and receive assistance, modeling, coaching, and support in doing so. 
  4. Ensure that the health and safety needs of participants who receive services are met consistently with COMAR, Center for Medicaid Services, and Board of Nursing regulations, accreditation standards, and agency policies and procedures.
  5. Report and investigate all incidents as required by the MD Department of Health and the Agency.
  6. Ensure that Participant Plans are written accurately and implemented. 
  7. Write outcomes, goals, and strategies as part of the Person-Centered Planning process and ensure that progress is being made toward the achievement of those outcomes, goals, and strategies.
  8. Ensure that medical services for participants who receive services are completed as required.
  9. Hire, manage, evaluate, supervise, and develop Direct Support Professionals.
  10. Conduct, and document, one-on-one supervision meetings with staff at least once per quarter.
  11. Conduct, and document, house meetings that include staff and participants who receive services at least monthly.  
  12. Work cooperatively with intra and extra-agency professionals and stakeholders to implement the agency’s mission, including supporting participants who receive services in their employment aspirations. 
  13. Use the agency’s database/system of records keeping ensuring accurate and timely input of data and information.
  14. Ensure that employees are scheduled for shifts, that staff are using the agency's time management system, and avoid excessive use of overtime.
  15. On a rotating basis, be on-call 24 hours a day for 7 days, and manage all calls made to the on-call emergency service.
  16. Conduct quarterly home visits and ensure follow-up actions on identified issues are completed.
  17. Ensure that the agency’s vehicles are inspected as required. 
  18. To attend all meetings and training as required.
  19. To fulfill other job duties as assigned.

Non-Essential Job Duties:

·      To participate in the agency’s committees and work groups.

·      To assist with agency marketing efforts and other events.

Education And Experience: 

  1. Bachelor’s degree in human services, Social Work, Psychology, or a related field.
  2. Minimum of 3–5 years of experience in residential services or a group home setting, including supervisory roles.
  3. Familiarity with person-centered planning, developmental disability services, and state regulations governing group homes.

Working Conditions:

Some evenings and weekends are required.

Must have drivers license to conduct agency business, including the transportation of supported participants.

Must be available during scheduled work hours by use of a personal cell phone.

Must work effectively within a team environment.
