    Position Summary

    The Primary Care Physician is responsible for providing comprehensive adult primary care within AbsoluteCare s holistic, value based model. The role is structured intentionally around a low daily patient census ( patients per day) to support thorough, relationship centered care and improved clinical outcomes. The physician works within an integrated clinical team to address medical, behavioral, and social determinants of health, with a focus on prevention, continuity, and reduction of unnecessary utilization.

    Essential Responsibilities

    • Provide longitudinal primary care, including preventive services, chronic disease management, and acute care.
    • Apply a whole person, holistic approach to patient assessment and treatment planning.
    • Work collaboratively with behavioral health, pharmacy, nursing, and care coordination to support complex patients.
    • Develop individualized care plans grounded in evidence based practice with measurable outcomes.
    • Engage in interdisciplinary case discussions and quality improvement activities.
    • Maintain thorough and timely clinical documentation in accordance with organizational and regulatory standards.
    • Support the organization s value based care objectives through proactive, patient centered clinical practices.

    Qualifications

    • MD or DO with board certification or eligibility in Family Medicine or Internal Medicine.
    • Eligibility for Illinois medical licensure.
    • Experience with medically complex or underserved populations preferred.
    • Demonstrated commitment to outcomes driven, value based care.
    • Strong clinical judgment, communication skills, and ability to work in a team based model
