About the Role:

As a Nurse Practitioner, you will play a crucial role in delivering high-quality preventative care to residents in skilled nursing facilities. This hybrid role combines on-site visits with remote work from home, supported by a dedicated scribe team for efficient documentation. You will build strong relationships with the facility, maintain a patient-centered approach, and be a collaborative team player.

Responsibilities:

• Provide high-quality preventative care to residents in skilled nursing facilities through on-site visits and remote monitoring
• Partner with a dedicated scribe team for efficient documentation
