About the Role:

We are seeking an experienced Nurse Practitioner (NP) or Physician Assistant (PA) with a background in Geriatrics or Internal Medicine to deliver preventative care to residents in skilled nursing facilities. As a hybrid role, you will split your time between on-site visits and remote work from home, supported by a dedicated scribe team for efficient documentation.

Responsibilities:
  • Provide high-quality, patient-centered care to residents in skilled nursing facilities
  • Build strong relationships with facility staff and maintain a patient-centered approach
  • Collaborate with our experienced clinical team to ensure seamless alignment with facility processes and protocols
  • Conduct on-site visits and remote work from home, supported by a dedicated scribe team for efficient documentation
  • Maintain accurate and timely documentation of patient care and outcomes
Requirements:
