About the Role:

The Case Manager RN (FT-Days) plays a critical role in coordinating and managing patient care to ensure optimal health outcomes. This position involves comprehensive assessment, planning, implementation, and evaluation of individualized care plans for patients across various healthcare settings. The Case Manager RN acts as a liaison between patients, families, healthcare providers, and insurance companies to facilitate effective communication and resource allocation. By monitoring patient progress and addressing barriers to care, the Case Manager RN helps reduce hospital readmissions and promotes efficient use of healthcare services. Ultimately, this role supports patients in navigating the healthcare system while advocating for their needs and improving overall quality of care.

Minimum Qualifications:

  • Current and valid Registered Nurse (RN) license in the United States.
  • Bachelor of Science in Nursing (BSN) or equivalent nursing degree.
