Revive Motion Clinic is seeking a dedicated and experienced Physical Therapist to join our dynamic team on a full-time basis in our Belleville, New Jersey outpatient office. In this role, you will utilize your clinical expertise to develop and implement individualized treatment plans aimed at enhancing patients' physical capabilities and improving their quality of life. This position offers a competitive salary ranging from USD 100,000 to 120,000 annually.

The physical therapist role is based in our outpatient office in Belleville, New Jersey, where you will work collaboratively with a team of healthcare professionals in a supportive and engaging environment. Your primary responsibility will be to assess, treat, and guide patients towards their recovery in a setting that emphasizes teamwork and patient-centered care. Our facility is equipped with state-of-the-art therapy tools and resources to help you deliver exceptional care to our clients.

• Direct and manage physical therapy treatment with a clear focus on preventing disability, relieving pain, and improving muscle function in alignment with each patient's plan of care.
• Provide supervision and guidance to physical therapy assistants to ensure effective and appropriate treatment implementation.
• Conduct thorough evaluations using diagnostic tests to assess each client's functional abilities, identifying areas for improvement.
• Analyze home environments and recommend necessary modifications to enhance independence and safety for clients.
• Administer prescribed physical therapy services in the home setting, performing therapeutic exercises, traction, and manual treatments.
• Document and maintain accurate clinical records, including initial evaluations, progress notes, and discharge summaries, in accordance with regulatory standards.
• Participate in interdisciplinary team meetings, providing insights and recommendations to contribute to care planning and evaluation.
• Monitor and report changes in client conditions, suggesting updates to the treatment plan as needed.
• Educate clients and their families about the therapies and rehabilitation tools to ensure optimal adherence and recovery outcomes.
• Complete additional tasks as assigned to support overall operational goals and promote effective patient care.

You will work primarily with a diverse patient population consisting of adults and seniors experiencing varying levels of physical dysfunction due to orthopedic injuries, neurological conditions, post-surgical recovery, and other rehabilitative needs. Our patient-focused approach ensures that all services are tailored to meet the unique requirements and circumstances of each individual, fostering a supportive and empathetic healing environment.

• A Bachelor's Degree in Physical Therapy from an accredited institution.
• A current state license to practice as a Physical Therapist in New Jersey.
• At least one year of experience in a physical therapy role, particularly in an acute care or home care environment.
• Current CPR certification and ability to maintain eligibility.
• A valid driver’s license and proof of automobile insurance to facilitate home visits when necessary.
• A proactive commitment to promoting clinical excellence and a strong passion for patient care.

While the minimum requirement is one year of experience, candidates with extensive backgrounds in multidisciplinary team settings or those who have managed complex cases in home health care will have an advantage. Familiarity with electronic health record systems is also preferred, as documentation is a vital component of this role.
