Position Overview

Revive Motion Clinic is seeking a dedicated and compassionate Physical Therapist to join our dynamic team on a full-time basis. This role involves delivering high-quality rehabilitation services directly in the homes of our patients in Oakland, California. As a critical member of our healthcare team, the Physical Therapist will be responsible for evaluating patients, designing personalized treatment plans, and executing interventions that promote recovery and enhance mobility.

Work Environment

As a Physical Therapist at Revive Motion Clinic, you will primarily work onsite in the comfortable, welcoming environments of our patients’ homes. The home care setting allows us to deliver tailored therapy in a familiar atmosphere, improving engagement and outcomes. This full-time position requires approximately 40 hours of work per week, with occasional variable weekends.

Key Responsibilities
  • Conduct comprehensive assessments of patients' physical capabilities and functional limitations, developing individualized treatment plans to address their unique needs.
  • Implement therapeutic exercises and modalities to facilitate rehabilitation, regularly re-evaluating patient progress and adjusting plans as necessary.
  • Collaborate with healthcare providers and interdisciplinary team members to ensure seamless patient care and continuity, providing updates and insights into patient statuses.
  • Educate patients and their families on treatment protocols, lifestyle modifications, and home-based exercises to promote autonomy in recovery.
  • Maintain meticulous patient records that comply with state regulations and facility protocols, documenting treatments, plans, and charges efficiently.
  • Engage in community outreach efforts, program development, quality improvement initiatives, and mentorship of therapy assistants, enhancing our service offerings.
  • Participate in team meetings and ongoing professional development opportunities, ensuring you are current with the latest evidence-based practices in physical therapy.
Patient Population / Client Focus

You will work primarily with a diverse patient population, including individuals recovering from orthopedic surgeries, those managing chronic pain, and patients living with disabilities. Providing compassionate care and empathetic communication with patients across various ages and cultural backgrounds is paramount to your role.

Qualifications
  • Bachelor’s Degree in Physical Therapy from an accredited institution.
  • Current state license in Physical Therapy and BLS certification are mandatory.
  • Demonstrated ability to communicate effectively, both verbally and in writing, with patients, families, and healthcare colleagues.
  • Proficiency in documentation and electronic health record systems.
  • Strong organizational skills with the ability to manage patient schedules effectively.
Preferred Experience

While not required, candidates with prior experience in home health settings or those who have worked with diverse patient populations will be given preference. Familiarity with contemporary rehabilitation techniques and outcomes measurement tools will enhance your effectiveness in this role.
