This full-time position is based in our outpatient office in Skokie. The setting is dynamic and patient-focused, promoting collaboration among healthcare professionals. Your regular schedule will be Monday through Friday, allowing for a balanced work-life routine. The duties of the Physical Therapist at Heal & Revive include:

• Designing and directing comprehensive physical therapy treatment plans aimed at preventing disabilities and relieving pain while optimizing muscular function.
• Conducting thorough evaluations of patients’ functional levels through diagnostic assessments and implementing individualized treatment strategies accordingly.
• Collaborating with physical therapy assistants to ensure the effective execution of therapy sessions and adherence to care plans.
• Assessing patients' home environments to recommend adjustments that promote rehabilitation and independence.
• Administering manual therapeutic exercises, and training patients in engagement with non-manual exercises and functional activities.
• Utilizing specialized techniques such as traction and massage, as well as physical agent modalities to enhance recovery.
• Evaluating, fitting, and adjusting orthotics and prosthetic devices, providing recommendations for modifications to physicians as necessary.
• Maintaining accurate clinical records, including initial evaluations, progress notes, and discharge summaries, in compliance with OASIS standards.
• Engaging with interdisciplinary teams during case conferences to devise and evaluate holistic treatment plans for clients.
• Training patients and their families in using therapy devices and understanding the overall therapy program.

Your primary focus will be on managing a diverse patient population that may include individuals recovering from surgery, those with chronic conditions, or patients needing rehabilitation after injury. The clinic emphasizes a personalized approach to treatment, ensuring that care is tailored to the specific needs of each patient for optimal recovery outcomes.

To be considered for this role, candidates must meet the following qualifications:

• A Bachelor’s Degree in Physical Therapy from an accredited institution.
• Active Physical Therapist licensure in the state of Illinois, maintained in good standing.
• Current CPR certification and a valid driver’s license with automobile insurance.
• A strong commitment to clinical excellence and a patient-centered approach in all aspects of care.
• Outstanding interpersonal skills, with a positive attitude and ability to work collaboratively in a team environment.
• High energy level, motivation for continuous learning, and exceptional communication capabilities.

While not mandatory, the following experience will be an advantage for candidates:

• One year of experience as a Physical Therapist in acute care or home care settings.
• Proficiency in managing patients through various stages of recovery from diverse physical conditions.
• Experience with electronic health record (EHR) systems for streamlined documentation and patient management.

The salary for this role ranges from USD 90,000 to 100,000 annually, commensurate with experience.
