Key Responsibilities

• Conduct thorough psychosocial assessments to identify clients' mental health needs, strengths, and available resources.
• Provide individual, family, and group therapy using recognized evidence-based therapeutic modalities like cognitive-behavioral therapy (CBT), trauma-focused therapy, and solution-focused therapy.
• Design and implement individualized treatment plans tailored to each client’s unique challenges and goals.
• Actively collaborate with an interdisciplinary team of psychiatrists, psychologists, and healthcare providers to ensure coordinated and effective care.
• Facilitate group therapy sessions, psychoeducational classes, and support groups that empower clients through shared experiences and learning.
• Provide crisis intervention and risk assessment, offering clients safety planning as needed.
• Maintain comprehensive, accurate documentation of client progress and treatment outcomes in accordance with ethical and legal standards.

Patient Population / Client Focus

As a Licensed Clinical Social Worker at Western Mind Care Hub, you will work primarily with clients in hospice care who are navigating mental health challenges brought on by terminal illness, grief, and adjustments to life-altering scenarios. Your understanding of complex emotions associated with end-of-life issues will be crucial in providing support to both clients and their families, fostering a healing environment that honors their experiences.

Qualifications

• Master's degree in Social Work from an accredited program.
• Current, active licensure as a Licensed Clinical Social Worker (LCSW) in the state of California.
• Minimum of three years of experience providing mental health services in an outpatient or related setting.
• Demonstrated expertise in evidence-based therapeutic approaches and treatment strategies relevant to mental health care.
• Exceptional interpersonal skills with a strong ability to communicate effectively with clients and team members.
• Skilled in conducting assessments and developing actionable treatment plans that align with client goals.

Preferred Experience

• Experience working with a geriatric population or individuals facing terminal illnesses is highly desirable.
• Proficient with telehealth platforms and digital documentation systems to facilitate remote therapy sessions.
• Knowledge of community resources and the ability to connect clients with additional support services as necessary.

Compensation and Benefits

The annual salary for this position ranges from USD 80,000 to 100,000, commensurate with experience, plus opportunities for performance-based bonuses. We also offer a comprehensive benefits package that includes:
