Job description

About Town Square Health
Town Square Health is reimagining what healthcare can be. We're building a first-of-its-kind value-based care model with a singular mission: to set the gold standard for how Americans experience healthcare. Our primary care locations offer comprehensive primary care with coordinated specialty support for Medicare-eligible patients, and we're growing fast, with expansion into multiple markets on the horizon. If you're bold, collaborative, and driven to make a real difference in people's lives, we'd love to have you on our team!
The Opportunity
The Regional Broker Relations Manager serves a key position within Town Square Health's growth team as we launch our comprehensive primary care services for older adults in Western New York.
This role builds and maintains relationships with independent insurance brokers and payer-side sales teams across Western New York to ensure they are knowledgeable about Town Square Health's physicians, services, and care model when advising Medicare-eligible clients in selecting a provider.
The position also serves as the internal subject matter expert on Medicare and Medicare Advantage plan structures, helping train Town Square Health's staff so they can speak knowledgeably with brokers, payers, and the community.
You'll have the opportunity to:
  • Build the broker and payer relationship infrastructure for a brand-new primary care organization in Western New York
  • Serve as Town Square Health's internal Medicare and Medicare Advantage subject matter expert
  • Shape how brokers, payers, and community members understand and access our care model
  • Work at the intersection of healthcare, community relations, and market growth in a high-visibility, field-based role

What You'll Do
Broker & Payer Relationship Building
  • Build and maintain relationships with independent brokers, agencies, and FMO/IMO networks operating in Western New York, ensuring they know Town Square Health's provider locations and services
  • Develop relationships with payer-side sales and account teams across the Medicare Advantage plans Town Square Health participates with or is in-network for, keeping them current on our offerings, capacity, and any service line changes
  • Act as a trusted, responsive point of contact for brokers' and payers' questions about Town Square Health, including which plans we are in-network with, what services are offered, and how to help a client access care
  • Represent Town Square Health at local broker meetings, payer trainings, community health fairs, and industry events to raise visibility among the broker and payer community
  • Track which brokers and payer teams have been engaged, and identify gaps in awareness or coverage across the region
  • Monitor and address non-compliant broker activity

Provider & Network Education
  • Maintain up-to-date, accurate materials describing Town Square Health's provider network, locations, services, and plan affiliations for use with brokers and payer partners
  • Coordinate with Town Square Health leadership and clinical and operations teams to stay current on new services, provider additions, or network changes that brokers and payers should know about
  • Ensure broker- and payer-facing communications are accurate, appropriately scoped, and compliant with CMS rules and regulations

Internal Subject Matter Expertise & Training
  • Serve as Town Square Health's internal expert on how Medicare and Medicare Advantage plans work, including eligibility, enrollment periods, plan types, and how beneficiaries typically choose providers within a plan's network
  • Design and deliver training for Town Square Health's outreach, community engagement, and member-facing staff so they can confidently discuss Medicare and MA basics with brokers, payers, and community members
  • Monitor CMS guidance and Medicare/MA industry changes relevant to how beneficiaries access providers, translating updates into practical briefings for internal teams
  • Develop internal job aids, FAQs, and reference materials on Medicare/MA topics to support consistent, accurate messaging across the organization

Market Insight & Reporting
  • Monitor the regional broker and payer landscape in Western New York, including key firms, FMOs/IMOs, and payer sales teams active in the market, to inform outreach priorities
  • Provide regular reporting to leadership on broker and payer engagement activity, relationship health, and awareness gaps across the territory
  • Partner with marketing and community relations to localize broker- and payer-facing materials for the Western New York market

Who You Are
You're a relationship builder who knows the Medicare and Medicare Advantage landscape well enough to be a trusted resource for brokers, payers, and internal teams alike. You're comfortable in a field-based role, energized by getting out in the community, and disciplined enough to track your relationships and report on what's working.
You communicate clearly about complex topics, you don't need a lot of oversight to stay on track, and you take pride in being the person others come to when they have a question they can't answer themselves.
Most importantly, you believe healthcare can be better - and you want to be part of the team proving it.
Qualifications
Required
  • Bachelor's degree in Business, Healthcare Administration, Marketing, or equivalent professional experience
  • 5-7+ years of experience in healthcare provider relations, community outreach, or Medicare/Medicare Advantage-adjacent roles (e.g., at a health plan, brokerage, FMO/IMO, or provider organization)
  • Solid working knowledge of how Medicare and Medicare Advantage plans function, including enrollment periods, plan types, and provider network structures
  • Demonstrated ability to build and sustain professional relationships with external partners (brokers, payers, community organizations)
  • Strong presentation and training/facilitation skills
  • Willingness and ability to travel regularly throughout Western New York (valid driver's license and reliable transportation required)

Preferred
  • Prior experience working with or alongside FMOs/IMOs, independent broker networks, or payer sales teams in a provider-relations or liaison capacity
  • Experience training non-sales staff (e.g., community outreach, member services) on Medicare/MA fundamentals
  • Existing relationships with brokers, agencies, or payer teams within the Western New York market
  • Familiarity with CRM or relationship-tracking tools

What We Offer
  • Salary range of $115,000-130,000/yr
  • Comprehensive benefits package (medical, dental, vision, 401K)
  • Opportunity to build something from the ground up
  • Direct impact on how Medicare-eligible patients in Western New York access and experience care
  • High visibility and influence in a growing organization
  • Collaborative, mission-driven team
