Job description

This is a founding clinical role. You will not inherit a workflow; you will help build one.

The Behavioral Health Care Manager (BHCM) is the clinical core of Cabana's Collaborative Care program. In this part-time-to-full-time role, you will partner directly with our Clinical Director to launch Cabana's CoCM model at our first partner sites in Louisiana. You will carry an active patient caseload, manage the CoCM registry, coordinate with a remote psychiatric consultant, and help design the clinical workflows, documentation standards, and engagement protocols that will define how Cabana delivers care at scale.

This role is ideal for a seasoned licensed clinician who is excited by the intersection of direct clinical care and program development.

Annual salary for this role is $79,000 - $90,000

What you will do

Direct Patient Care

  • Carry an active caseload of patients enrolled in the CoCM registry across partner sites (primary care, urgent care, and/or emergency settings)
  • Deliver brief, evidence-based interventions using Collaborative Care principles — including Problem-Solving Treatment (PST), Motivational Interviewing (MI), Cognitive Behavioral Therapy (CBT), and Behavioral Activation
  • Conduct structured screenings using PHQ-9, GAD-7, Columbia Suicide Severity Rating Scale (C-SSRS), and other validated tools
  • Monitor patients for changes in clinical symptoms, treatment adherence, and side effects; adjust care plans accordingly
  • Complete relapse prevention planning with patients who reach remission
  • Educate patients about their behavioral health condition, treatment options, and self-management strategies

Registry & Care Coordination

  • Maintain and manage the CoCM patient registry, tracking weekly PHQ-9/GAD-7 scores and clinical status inside the partner EHR (Epic, Cerner, or equivalent)
  • Participate in weekly caseload consultation with the remote Psychiatric Consultant; implement care plan changes based on consultation recommendations
  • Facilitate warm hand-offs from medical providers and Care Navigators, ensuring seamless enrollment from the point of identification
  • Coordinate step-up care for patients who are not improving, including appropriate referrals for higher levels of care or specialty services
  • Identify social determinants of health (SDOH) barriers and collaborate with Care Navigation to connect patients to community resources
  • Document all patient encounters, progress notes, and care plan updates in the partner EHR in a timely and compliant manner

Program Development & Workflow Design

  • Partner with the Clinical Director to develop, test, and refine CoCM workflows, intake protocols, and documentation standards for Cabana's first partner sites
  • Help design the warm hand-off script, registry management process, and escalation pathways used across future site launches
  • Contribute to the development of training materials, onboarding guides, and clinical SOPs as the program scales
  • Provide feedback on the integration of Cabana Live (digital app) with clinical workflows, including how engagement data informs clinical decision-making
  • Participate in pilot quality review meetings, bringing a clinical lens to program performance data

Requirements

Required

  • Active Louisiana licensure as an independent or associate-level clinician: LCSW, LPC, LMFT, or equivalent
  • Master's degree in Social Work, Counseling, Marriage and Family Therapy, or a related clinical field from an accredited program
  • Minimum 3-5 years of direct clinical experience post-graduation
  • Working knowledge of the Collaborative Care Model (CoCM) and measurement-based care principles
  • Experience administering and interpreting validated screening tools (PHQ-9, GAD-7, C-SSRS)
  • Comfort working inside electronic health records (EHR) for documentation and registry management
  • Strong written communication skills — accurate, timely clinical documentation is a core function of this role
  • Experience moderating groups (group therapy or other forms of groups)

Strongly Preferred

  • Experience in a CoCM, integrated behavioral health, or primary care behavioral health setting
  • Proficiency in evidence-based brief intervention modalities: PST, MI, CBT, Behavioral Activation
  • Experience managing a caseload using a population health or registry-based approach
  • Familiarity with billing and documentation requirements for CoCM (CPT 99492, 99493, 99484) or BHI codes
  • Comfort working across settings — clinic and/or telehealth
  • Experience identifying and addressing SDOH barriers in a clinical context
  • Bilingual in English and Spanish (a plus, not a requirement)
