Job description

ESSENTIAL FUNCTIONS:

The Behavioral Health (BH) Care Managers will build relationships with patients, assess, and identify patient needs, and connect them with community-based care teams. BH Care Managers will serve as a bridge between the patient, the medical system, and community-based organizations by building trusting relationships with community members served by the program. BH Care Managers will also help in identifying barriers to accessing quality care, work with individuals to overcome these barriers, and provide relevant referrals.

Community-based Behavioral Health Care Managers are employed by WSHEC participating community mental health centers.

RESPONSIBILITIES:

  • Meet with target patients and begins to build a trusting relationship. Conduct crisis assessment if applicable
  • Identify specific patient-identified needs and seek to begin to address them during the initial visit if possible
  • Engage the enrollee in the community setting to facilitate continuity of care and enrollment into enabling services
  • Work with enrollees to improve condition-specific self-management
  • Periodic rescreen of enrollees using the health risk screening tool to determine progress in addressing social determinants of health and identify any emerging issues
  • Participate in case reviews with other care team members (nurses, behavioral health providers, PCPs, pharmacists, social workers, care managers of high-risk enrollees as appropriate) of enrollees who are not making progress toward self-management goals

QUALIFICATIONS:

  • Bachelors level required; Masters preferred
  • 2-3 years working in mental health or related field
  • Demonstrate ability to collaborate and communicate effectively in a team setting.
  • Punctual, reliable, and willing to learn
  • Ability to maintain effective and professional relationships with patients and other members of the care team.
  • Ability and opportunity to work flexible hours.
