Registered Nurse (RN) - Transitional Care Unit
Job Type :Full Time Variable 8- & 12-hour shifts and every other weekend and holiday rotation No on-call

Up to $20,000 bonus for eligible candidates

Our Transitional Care Unit (TCU) is a 30-bed, hospital-based, short-term rehabilitation unit located at Memorial Medical Center. Patients receive daily skilled nursing care along with a minimum of two hours per day of physical, occupational, and speech therapy, supporting a safe and efficient transition back home.

TCU features 24 patient rooms, including 18 private and 6 semi-private accommodations. Patients typically stay 10-12 days to recover strength and independence following surgery or acute illness. Common care needs include post-operative recovery, wound care, IV therapy, chronic disease management, pain management, and education for new diagnoses.

Staffing & Support:

  • Day Shift (7A): Typically staffed with 3-4 licensed nurses (mix of RNs and LPNs based on census) and 2-3 CNAs
  • Night Shift (7P): Typically staffed with 2-3 licensed nurses (RN/LPN mix) and 2 CNAs
  • Average daily census up to 24 patients

This unit offers a team-oriented environment focused on rehabilitation, patient education, and helping individuals regain independence and return home safely.

How you'll contribute

A Registered Nurse (RN) who excels in this role:

  • Assesses needs of residents and establishes priorities for resident care based on medical and nursing diagnoses.
  • Reassess and evaluate changes in resident's condition as well as reports and documents these changes.
  • Assists with the completion of the Minimum Data Set (MDS).
  • Assesses learning needs of residents and/or families.
  • Provides teaching based on standardized teaching protocols in conjunction with the resident's plan of care.
  • Evaluates outcomes of teaching residents and/or families.
  • Documents pertinent data and/or relevant information in resident's medical record.
  • In the absence of D.O.N., provides supervisory responsibilities including discipline, assignments, scheduling and evaluation of job performance.
  • Maintains confidentiality regarding resident information.
  • Accurately initiates and transcribes physicians orders in a timely manner.
  • Administers medications in accordance with nursing policies and procedures and all applicable regulations.
  • Assumes responsibility for initiating and monitoring IV therapy, etc.
  • Participates in reviewing/revising unit philosophies and objectives, policy and procedure manuals, etc.
  • Follows care plans to ensure that resident care needs are met.
  • Demonstrates ability to prioritize and organize work with flexibility. Completes assigned work during shift in a timely, accurate manner.
  • Establishes a plan of care on new admissions and updates it with changing needs/and/or conditions of the residents.

What we're looking for

Applicants should have a current state RN license and possess a bachelor's degree from an accredited nursing school. Additional requirements include:

  • Basic Life Support certification is required within 30 days of hire.

  • CPR Certification

  • Two years of nursing experience preferred

  • One year of long-term care experience preferred

  • LTC experience preferred
