Job Title: Part-Time Portal Management Assistant – Home Health Agency
We’re seeking a detail-oriented and tech-savvy Portal Management Assistant to join our home health team on a part-time basis. This role is responsible for managing the electronic medical records (EMR) portal, ensuring timely and accurate intake of referrals, document uploads, communication tracking, and general portal organization.
Key Responsibilities:
• Upload and organize clinical documentation in the EMR
• Monitor referral portals and ensure timely intake of new patients
• Track communication between referral sources and clinical staff
• Maintain compliance with documentation timelines and standards
• Collaborate with intake, scheduling, and clinical teams to ensure smooth transitions
Qualifications:
• Experience with home health EMRs or similar systems preferred
• Strong attention to detail and organizational skills
• Ability to work independently and communicate effectively
