I hereby release and agree to hold all parties, including Production Saw & Machine, their respective owners, Boards, officers, administrators, managers, employees, and agents harmless for any and all liability arising from or related to the testing, my consent and/or refusal to consent hereto.My agreement to the terms of the Substance Abuse Screen Authorization and Continuing Consent does not guarantee me employment with Production Saw & Machine.
