POSITION SUMMARY:
The Licensed Clinical Social Worker (LCSW)/Licensed Professional Counselor (LPC) is a part of the patient care team whose primary purpose is to work (Hybrid- partially remote) in conjunction with the health services within the health center in order to provide assessment, diagnosis and treatment planning of behavioral health to children, adolescents, adults, and families in order to improve psychosocial functioning.
RESPONSIBILITIES:
• Assists in the development of a system to assess and intervene with high-risk patients
• Develops overall resource requirements to meet identified needs
• Develops and implements policies, procedures and guidelines
• Utilizes current research findings to support clinical social work services best practices on an ongoing basis
• Participates in the delivery of care as scheduled
• Performs and documents patient's screenings, psychosocial assessments, progress and referral information
• Assists in the process of advocacy for patient's access to resources through community programs and coaching to the patients, when appropriate
• Communicates treatment plan to patients, families and appropriate staff
• Collaborates and coordinates with the caregiver, medical provider, service assessment, case management, home health care and all other disciplines necessary to provide the best care to patients
• Provides group and individual counseling to patients on related psychosocial issues
• Demonstrates a commitment to service, organization values and professionalism through appropriate conduct and demeanor at all times.
• Maintains confidentiality and protects sensitive data at all times.
• Adheres to organizational and department specific safety standards and guidelines.
• Works collaboratively and support efforts of team members
• Will not allow any organizational practice, activity, decision or circumstance which is unlawful, negligent, contrary to the mission, vision or policies of the Mid-Delta Health Systems.
QUALIFICATIONS:
• Must have the ability to work effectively independently and in a team environment
• Excellent interpersonal skills
• Excellent verbal and written communication skills
• Requires professionalism and ability to deal effectively with patients, families, caregivers and a variety of internal and external resources
• Master's Degree (MSW) in social work from an accredited institution required
• Clinical Social Worker licensed by the State of Arkansas
• Minimum of three years of experiences in a healthcare setting required
• Bilingual in English/Spanish preferred
PATIENT CENTER MEDICAL HOME:
PCMH transformation is successful when there is support from a clinician lead. Their support sets the tone for how the practice will function as a medical home. The intent is to ensure that the practice has clinician and leadership support to implement the PCMH model and to acknowledge the role of staff in the practice's everyday operations.
SCOPE OF SERVICES:
• Medical Services- The Licensed Clinical Social Worker/Licensed Professional Counselor shall be available at the designated Facilities during their Hours of Operation as shall be necessary and practical for the prompt delivery of the Services. All medical services shall be provided in accordance with the Licensed Professional Counselor/Licensed Clinical Social Worker's specialty and MDHS's Scope of Project as it is defined for purposes of coverage under the Federal Tort Claims Act.
• MDHS Facilities- The Licensed Clinical Social Worker/Licensed Professional Counselor shall be assigned to provide services at one or more Facilities at MDHS sole discretion. In addition, from time to time and at MDHS's sole discretion, the Licensed Clinical Social Worker/Licensed Professional Counselor may be asked to provide services at additional or fewer MDHS Facilities.
SALARY POSITION:
The salary for this position is negotiated between the individual and the Chief Executive Officer within the guidelines of the approved MDHS salary plan.

This is a FLSA Exempt position.
