LTSS Service Care Manager Job Type: Contract â€" 5 Months Pay Rate: $____/hour Schedule: Mondayâ€"Friday, 8:00 AMâ€"5:00 PM EST Location: Remote â€" North Carolina, Region 4 Preferred Candidates: Durham & Wake Counties Start Date: ASAP Potential for Extension/Conversion: Yes A-Line Staffing Solutions is currently recruiting for an LTSS Service Care Manager position supporting members throughout North Carolina. This is a primarily remote opportunity with face-to-face home visits and occasional in-person team meetings.

Job Summary

The LTSS Service Care Manager will assist in developing, assessing, and coordinating holistic care management activities to support quality, cost-effective healthcare outcomes. This position will work with long-term care members to develop personalized service plans, coordinate services, connect members with providers and community resources, and educate members and their families/caregivers on available benefits and services. This role works as part of a collaborative care team alongside Utilization Management, Quality, Transition of Care, Care Managers, Coordinators, and Navigators.

Responsibilities

Evaluate member needs, available resources, and appropriate care options. Assist with developing and maintaining long-term care/service plans. Identify providers, specialists, and community resources needed to support members. Coordinate between members, families/caregivers, and healthcare providers. Monitor member care plans, status, and outcomes and recommend changes based on identified needs. Provide resource support related to housing, employment, independent living, participant direction, foster care, and other member needs.

Interact with long-term care providers and community partners to ensure member needs are met. Maintain accurate documentation of member information and care management activities. Ensure documentation complies with applicable state, federal, and third-party payer requirements. Conduct home and/or other site visits to assess member needs and collaborate with providers and partners. Educate members and families/caregivers regarding healthcare instructions, service options, referrals, procedures, and benefits. Provide feedback to leadership regarding opportunities to improve quality and cost-effective service delivery. Perform other duties as assigned.

Requirements

Bachelor's degree and 2â€"4 years of related experience, or equivalent applicable experience. RN or LCSW license required. Minimum of 2 years of LTSS and/or HCBS coordination, care delivery monitoring, or care management experience. Minimum of 2 years of in-home clinical experience. Experience in social work, geriatrics, gerontology, pediatrics, human services, or a related area. Strong clinical documentation and computer skills. Ability to work independently and manage a caseload autonomously. Reliable access to personal transportation. Valid driver's license required due to member home visits. Must be comfortable conducting face-to-face home visits. Prior managed care experience is preferred.

Ideal Candidate

The ideal candidate has strong care management or in-home clinical experience, is comfortable working independently, and can effectively coordinate care across members, families, providers, and community resources.

Work Environment

This is primarily a remote position, but the role includes face-to-face member home visits and may require more than 50% travel, depending on caseload and member needs. Occasional in-person team meetings may also be required. The team operates in a collaborative and supportive environment focused on quality care management, strong coordination, and positive member outcomes.
