Licensed Practical/Vocational Nurse (LPN/LVN) - Home Health
Job Summary
We're looking for a skilled and compassionate LPN/LVN to provide in-home nursing care under the supervision of a Registered Nurse (RN). You'll support patients with daily care needs, monitor their condition, and help ensure safe, high-quality home health services.
Location: Central
Responsibilities
  • Provide nursing care according to the RN-developed Plan of Care.
  • Monitor and document changes in patient condition; report to RN/physician as needed.
  • Assist with activities of daily living to promote patient independence.
  • Educate patients and caregivers within LPN/LVN scope of practice.
  • Maintain accurate EMR documentation.
  • Participate in case conferences and coordinate care with the home health team.
  • Follow infection-control, HIPAA, and company policies.

Requirements
  • Current LPN/LVN license in the state of practice.
  • Graduate of an accredited practical/vocational nursing program.
  • Minimum 1 year acute care experience preferred.
  • Current CPR/BLS certification.
  • Valid driver's license, reliable insured vehicle.
  • Strong communication, clinical judgment, and ability to work independently. Ability to pass required physical exams and annual PPD testing.
