Licensed Practical Nurse/LPN, Part Time, 0.8 FTE Night Shift, Milwaukee, WI  ( $3,200 Sign on Bonus)

Community Care is hiring Licensed Practical Nurses/LPNs who are committed to teamwork and providing great care to become a part of our compassionate caregiving team. We are offering rewarding opportunities to provide care and service in peaceful, tidy and a well-supplied residential setting that is home to a sister community we serve.  

The Licensed Practical Nurse/LPN provides nursing care and services for our residents. This is a part time opportunity on our night shift for Sacred Heart location at 1555 S. Layton Blvd, Milwaukee, WI 53215.

Licensed Practical Nurse/LPN Job Responsibilities:  

  • Work in collaborationwith other nursing staff to coordinate and provide direction of care during weekday operations.  

  • Serves as coordinator of care for scheduled weekend and holiday shifts.  

  • Ensures care and service delivery are in conformance with regulations, policies, procedures, and standards, established by Community Care, the Program for all-inclusive Care for the Elderly and the State of Wisconsin. 

Licensed Practical Nurse/LPN Job Requirements:  

  • Education:Diploma from accredited program in practical nursing required. 

  • Licensure: A current Wisconsin Licensed Practical Nurse/LPN license in good standing 

  • Certification: Current CPR/AED certification 

Licensed Practical Nurse/LPN Job Schedule: 

  • 10:45 PM – 6:45 AM 32 hours per week/  Every other weekend and every other holiday

  • Every other weekend 

  • Every other holiday
